Receive your companion airline ticket voucher in these 4 easy steps:

- Purchase a qualifying Black & Decker product between April 15 and June 30, 2007. (Review eligible products below or Print List)
- Complete the Redemption form in its entirety.
- Collect the original UPC scan code from product packaging, a copy of your sales receipt with the purchase date clearly circled, and a self-addressed return envelope.
- Mail redemption form, UPC, sales receipt, and self-addressed stamped envelope to: LVI Fulfillment Center, 5300 West Atlantic Avenue, #102, Delray Beach, Fl 33484. Allow
4-6 weeks to receive your Companion Airline Ticket Activation Voucher.
